Session Hangs When Creating A Purchasing Order

Symptoms
When attempting to Approve a Purchase Order, the following error occurs.
ERROR
-----------------------
FRM-92100: Your connection to the Server was interrupted.
This may be the result of a network error, or a failure on the Server.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. In Purchasing Responsibility > Purchase Order > Purchase Orders, create new PO, Save.
2. Click Approve Button, pop up the Approval window.
3. Click OK button, got following error:
FRM-92100: Your connection to the Server was interrupted.
This may be the result of a network error, or a failure on the Server.
