Competencies are not Appearing on the Employees Perfromance Review Page

Symptoms
On : 12.1.3 version, Appraisals
While adding competencies for the appraisal, the following error occurs:
"Error This competence already exists within the assessment"
STEPS
The issue can be reproduced at will with the following steps:
1.Responsibility Employee Self Service
2.Performance Management
3.Select appraisal : XXX Appraisal Template
4.Update the Appraisal : Initiator :YYYYYYYYYYYY; Main Appraiser :ZZZZZZZZZZ; Appraisal Date : DD-Mon-YYYY
5.Click Add Competencies
6.Error : error “This competence already exists within the assessment.”
