Why does Autocreate not create Approved Requisition line into a Purchase Order(PO) without returning error?

For example, when the Approved Requisition Line is Autocreated to PO the Requisition line cannot be created as the PO as it is returning an error message.

ERROR:

“This Document already exists. Please Choose the Add To document Action to add to this document”
