Submit request 'Delivery To' window is not displaying/behaving correctly

Problem Description
---------------------------------------------------
[In the Submit Request window] When clicking on Delivery Options button, it opens up 'Delivery To' window. This window is partially displayed and is non-stretchable and cannot be expanded. Unable to see OK, Cancel button and other fields.
