Wrong Employee In Action History When Approving & Forwarding A Req From Email Notification (Doc ID 1632312.1)

Goal

Why is the Action History showing the wrong employee when using the Approve and Forward in a Requisition email notification?

On 12.1.3 it is observed that intermittently an incorrect user was getting inserted in the Requisition Action History.  Although intermittent it was found to be related to the GUEST feature as follows: 

The Approver was using the GUEST user to access the email notification in order to  click on the "Please Click here to Respond" link from the  email notification body, which then takes them to the Requisition Details  page. There they can click the Approve button, etc.

In using the GUEST user on approval, the responder is set to GUEST resulting in the Forward row in the PO Action History being set to employee_id = 6 (GUEST  user_id).  The conflict is that there is also a user having the employee_id(person_id)=6 in the per_people_f table.   It seems that the problem is because the GUEST user (user_id = 6) when approving the Notification thru Outlook is populated as employee_id 6 in  the PO_ACTION_HISTORY table.  When viewed in the PO Action History view, we see a terminated employee tied to PERSON_ID 6 in the "Forward" action.
