TeleService: FRM-92101 When Trying to Open the Create Service Request or Contact Center forms (Doc ID 1632564.1)

Symptoms

After perform an upgrade from 12.2.2 to 12.2.3, when attempting to open the Create Service Request form (CSXSRISR) or the Contact Center form (CSCCCCRC), the following error occurs.

ERROR
-----------------------
FRM-92101: There was a failure in the Forms Server during startup.
This could happen due to invalid configuration.
Please look into the web-server log files for details.

oracle.forms.net.ConnectionException: Forms session <69> aborted:
unable to communicate to runtime process.
    at oracle.forms.net.ConnectionException.createConnectionException(Unknown Source)
    at oracle.fomrs.net.HTTPNStream.getResponse(Unknown Source)
    at oracle.forms.net.HTTPNStream.doFlush(Unknown Source)
    at java.io.DataOutputStream.flush(Unknown Source)
    at oracle.forms.net.StreamMessageWriter.run(Unknown Source)

(N) Service Requests > Create Service Request

  or

(N) Service Requests > Find Service Request:
        - Query an existing SR

  or

(N) Customer Management > Contact Center

  or

(N) Customer Management > Contact Center
      - Query a customer
      - Navigate to the Service Request tab and try to create a new SR
