Field Service Report Is Throwing Error In Mobile Field Service Wireless

When attempting to open Task Report setup from a user defined Task Report Template, the following error occurs.
Adobe Reader could not open 'signature(2).pdf' because it is either not supported file type or because the file has been damaged.
(For example, it was sent as an email attachment and was not correctly decoded).
1. Setup a Task Report Template and use the same CSFREPORT.rtf report file from the standard Task Report Template.
2. Set profile option: 'CSF: Default Task Report Template' to the user defined Template
3. Log in Field Service Wireless
4. Open Task
5. Try to open the Task Report
