Symptoms

Mixed up UI is seen after opening and closing "LOG and Notes" in the Service Request form.

Steps
User selects and opens a ticket in Service Request form.
User switches to "Task" tab in Service Request form.
User opens "Log and Notes" window from the active "Task" tab
User closes "log and Notes" window again
UI shows a mixture of "Workbench" and "Task" tab.
