Unable To Control A Patch Using Adctrl, Getting UNDEF For All Workers (Doc ID 1667094.1)

Symptoms

Patch is failing with error :

sqlplus -s APPS user/password = <PASSWORD> @<$AR_TOP>/12.0.0/patch/115/sql/<SERVICE NAME>

When opening adctrl to check the Patch status following error occures :
