Symptoms
In a Production 12.1.3 environment, when approving the "Leave Of Absence" (or other) workflow the following errors occur:

Error Message: ORA-06511: PL/SQL: cursor already open
Value Found: None
Remarks: Please use only the suggested values, making sure they are enclosed within quotes (For example "", "", and so on)
