Cannot Launch Forms after Applying Patch 18497540:R12.TXK.C

On Oracle Applications 12.2 version, Forms for App Technology:
Forms do not launch after applying patch 18497540:R12.TXK.C.
The issue can be reproduced at will with the following steps:
1. Apply Patch 18497540 - CONSOLIDATED FIXES FOR R12.TXK.C.DELTA.4 on a 12.2.0 instance.
2. Attempt to access forms.
3. NO errors occur as they just do not launch.
Noted that: http://host.domain:PORT/forms shows forms are running and
http://host.domain:PORT/forms/frmservlet - does bring up login page
