Using AME, Requisitions With A Manually Added Approver (via iProcurement) Are Getting Stuck In The Approval Process (Doc ID 1905401.1)

Symptoms

On 12.1.3 version using the Approval Management Engine (AME) for requisition approval routing, after patching delivering POXAMEPB.pls 120.41.12010000.9 when the user inserts an approver (via Manage Approvals button in iProcurement) after this inserted approver responds, the original next approver is not notified and the requisition is stuck IN PROCESS.   There are no workflow errors but the wfstat.sql output may show something similar to the following:

STEPS
The issue can be reproduced at will with the following steps:
1. In iProcurement create Requisition and using Manage Approvals insert approver
2. When the inserted approver is notified and responds the next approver is never notified
3. Requisition workflow stops and cannot continue
