TeleService: Error When Opening Service Request Form: "FRM-40831: Truncation Occurred ORGANIZATION_NAME" (Doc ID 1911267.1)

Symptoms

There is an error while trying to open the Create Service Requests form.
The name of the Operating Unit is longer than 60 characters.

Error:

FRM-40831: Truncation occurred: value too long for field ORGANIZATION_NAME.

Steps:

(R) Inventory
 - Create Operating Unit with a long name (i.e. "My Organization Medical Devices Worldwide Co., Ltd. WW OU")

(N) Service
 - Open SR form
 - In the Operating Unit (if the value is not defaulted), open the LOV and choose "My Organization Medical Devices Worldwide Co., Ltd. WW OU".
