Account Hierarchy Manager (AHM)

Symptoms
When attempting to launch Account Hierarchy Manager (AHM) it does not start and the following error occurs:
Error

Bad Request
The request could not be understood by server due to malformed syntax.
Also:
Error: The webpage cannot be found HTTP 400
