Symptoms

When attempting to launch Account Hierarchy Manager (AHM) it does not start and the following error occurs:

Error
-------
Bad Request
The request could not be understood by server due to malformed syntax.

Also:
Error: The webpage cannot be found  HTTP 400

Also:
Error: 400 Bad Request

1. General Ledger Super User
2. Navigate Setup > Accounts > Manager
