Still Getting Workflow Error WFENG_ITEM_UNIQUE Even When There Is Only 1 Subscription To The Business Event oracle.apps.per.irc.common.notifications (Doc ID 1924403.1)

Goal

There are no notifications being sent in iRecruitment as the error below is getting raised in Oracle Workflow:

Event Error Name: WFENG_ITEM_UNIQUE
Event Error Message: 3122: Duplicate item 'IRC_NTF/<nnnnn>' could not be created.
Event Error Stack:
Wf_Engine_CreateProcess(IRC_NTF, <nnnnn>, IRC_NOTIFICATION_PRC)
IRC_NOTIFICATION_WORKFLOW_PKG.launchNotificationsWorkflow(oracle.apps.per.irc.common.notifications, <alphanumeric code>)

According to the note below, the fix is to only have 1 enabled subscription to the business event oracle.apps.per.irc.common.notification:
<Note 1313111.1> - SYSADMIN User Receives Error Notification: 'An Error occurred in the following Event Subscription: Event Subscription'

But based on the results of the sql queries from the note above, there is only 1 subscription to the event. So why is the issue still happening in the instance?
