Symptoms
City jurisdiction is not found for 4 locations.

The zip code on the sample transaction is:

When you process with the customer Inner City checked, on the customer record, you are not getting a city line due to the zip code of 86351 that is attached to the Outer City jurisdiction, not the Inner City jurisdiction.
So, for not getting a rate for both Inner City check and not checked is correct because checking the 'Inner City' on the customer record is wrong due to the zipcode per CCH records is attached to '-2' (the Outer City juridiciton).
The '-2' city jurisdiction is only being looked at.
