Form and API sre Allowing Duplicated for Plan Details Organizations (Doc ID 1926871.1)

On 12.1 HRMS RUP5 version of Oracle Advanced Benefits,

Application is allowing the user to save duplicate rows in Plans form > Details button > Organizations tab.

Steps to Reproduce:
The issue can be reproduced at will with the following steps:

1. Go to responsibility US Super HRMS Manager.
2. Navigate to Total Compensation > Programs and Plans > Plans.
3. Query the plan in question and click the Details button.
4. Select the Organizations tab and enter the exact details twice.
