Symptoms
On : 12.1.3 and 12.2.3 version, Appraisals
When attempting to approve any request, the following error occurs.
ORA-06558: buffer in dbms_pipe package is full. No more items allowed
When queried db_pipes shows XXX_XXX pipe size with a high value.
STEPS
The issue can be reproduced at will with the following steps:
