Supplier survey gets error when the prospective supplier tries to register

"The webpage cannot be found" HTTP 400

The issue can be reproduced at will with the following steps:
1. Log in with the prospective supplier registration link
2. Try to respond to the Survey
3. After entering the responses click NEXT button and get the error
