Receipts Workbench Changes Are Not Getting Saved and Cannot Query System Options - ORA-01422: Exact Fetch Returns More Than Requested Number of Rows (Doc ID 1933741.1)

Symptoms

On : 12.1.3 version, Receipts Related

Attempting to Reverse a Receipt.
The application does not save the reversal, and keeps asking you if you want to save the changes.

Steps to Reproduce:
Responsibility: Receivables responsibility
Navigation: Receipts > Receipts
1. Query Receipt
2. Click on Reverse
3. Enter information
4. Save

FND Debug shows the following:

Also, queries in sqlplus using APPS synonym AR_SYSTEM_PARAMETERS are showing the information for all the operating units as if you were was querying the table AR_SYSTEM_PARAMETERS_ALL directly

Another possible symptom is that you cannot query in the System Options screen (Navigation: Setup > System > System Options). The query always returns a blank row
