ORA-06508: PL/SQL: could not find program unit being called in Package OE_Delayed_Requests_PVT Procedure LOGREQUEST (Doc ID 1938752.1)

Symptoms

On : 12.1.1 version, Product Utilities

After applying <Patch 17863140> (Latest Recommended Patch Collection for Oracle Purchasing),
when attempting to copy an order, the following error occured:
   ORA-06508: PL/SQL: could not find program unit being called in Package OE_Delayed_Requests_PVT Procedure LOGREQUEST

"Following <Note 1633568.1> - Order Import Error ORA-01403 no data found in Package OE_Holds_PUB Procedure evaluate_holds_post_write HOLDS, RDBMS <Patch 8639653> was applied.
(Order Management <Patch 7608581> mentioned in the note is already applied).
Then, when copying again, a new error message displays:
    ORA-01403: no data found in Package OE_Holds_PUB Procedure evaluate_holds_post_write

Support recommended <Patch 14040669>:R12.ONT.B with OEXPHLDB.pls 120.10.12010000.26
Per notes:
<Note 1614305.1> - ORA-01403 no Data Found In Package OE_Holds_PUB Procedure Evaluate_holds_post_write HOLD NOT APPLIED WITH CRITERIA
<Note 1508427.1> - When Entering Sales Order get ORA-01403: NO DATA FOUND IN PACKAGE OE_HOLDS_PUB PROCEDURE EVALUATE_HOLDS_PUB

After applying <Patch 14040669>:R12.ONT.B, now copy order errors with:
   The flexfield on this field contains a value set (GL_SRS_FLEXFIELD) that uses special or pair user-exit validation.
   This prohibits operation in the mode that optimizes performance over a wide-area-network.
   Request failed: Invalid request/action DR_COPY_OTM_RECORDS.

Instance is not using OTM.
Profile setting for 'Flexfields:Validate On Server' = Yes at Site level.

STEPS TO REPRODUCE
 
1. Order Management Responsibility
2. Sales Orders > Query Order
3. Actions > Copy > results in error
