Symptoms

On : 12.1.3

When trying to 'Self Schedule' a task in field service wireless a blank screen appears instead of the task getting scheduled.

These steps are followed to reproduce the error.

STEPS
-----------------------
 1. Login to Field Service Wireless.
 2. Select open tasks.
 3. Select any task in the list.
 4. Create a follow up task.
 5. Select option 'Self Schedule'. 
 6. A blank page  appears.
