Symptoms
On Oracle Applications 12.1.3 version,
when attempting to submit any request, request fails with Inactive No Manager Phase/Status Combination,
the following error occurs.
Inactive No Manager
The issue can be reproduced at will with the following steps:
1. Submit any request for example Active Users.
The issue has the following business impact:
Due to this issue, users cannot run any request.

Created Include Rule for Standard Manager.
