Error While Updating UDA Page In Supplier Registration Process To Remove Duplicate Records

Symptoms

On Release 12.1.3

When attempting to register a new supplier and trying to use UDA attributes, the following error occurs.

 ERROR
There is already a row of "Insurance(Autombile, Commercial General Liability & Workers Compensation Reqd)" with "Insurance Type" as "Commercial General Liability". Please enter a distinct value for "Insurance Type".
oracle.jbo.TooManyObjectsException: JBO-25013: Too many objects match the primary key oracle.jbo.key[0]

STEPS TO REPRODUCE
The issue can be reproduced at will with the following steps.
1. Using SLM Responsibility, Go to Suppliers > Profile Management and click on 'New Supplier Request'.
2. Enter the mandatory details and select 'Register' from Actions and click on Go.
3. See the below error on top of the page: Error
