Symptoms
Intermittently when attempting to forward a requisition using the Forward Documents form, the requisition gets stuck in process and the following errored activities are generated:

• Activity Display Name = Update Action History (Forward)
• RESULT = #EXCEPTION
• ERROR_NAME = -6512
• ERROR_MESSAGE = ~ORA-06512: at line ~
• ERROR_STACK = Wf_Engine_Util.Function_Call(PO_APPROVAL_LIST_WF1S.UPDATE_ACTION_HISTORY_FORWARD, REQAPPRV, 196435-995956, 265952, RUN)
The issue can be reproduced at will with the following steps:
1. Navigate to Management > Forward Documents
2. Find documents to be forwarded
3. Select documents
4. Specify the new approver in New Approver field
5. Save
