Error when Receiving a PO "There has been a database insert error, please contact your dba or Oracle"

Symptoms
User try to receive an item from a purchase order and get the following error message : There has been a database insert error, please contact your dba or Oracle Support.
This is the only item from this purchase order with this error message.
This is a WMS-enabled warehouse and the reception is done thru mobile application.
ERROR
It seems to happen only with lots which have been created before upgrading to R12.
Lots which have origination date and created after upgrade to R12 don't have this issue.
STEPS
The issue can be reproduced at will with the following steps:
1. In mobile, use PO Receiving function.
2. Enter PO number
3. Enter Item
4. Enter LPN
5. Enter Qty
6. Enter Lot
7. Enter Lot Qty
Click on Next Item
Then you get the error message.
