12.2.3 View Action History Results are Incorrect (Doc ID 1955816.1)

Symptoms

On 12.2.3 instance there is an issue with Action History not recording correctly when compared to previous 11i version.

When navigating to "Inquire - View Action History" on an approved Purchase Order the following cases are seen:

1. Self Approved > Only shows 'Submit' in R12 > whereas in 11i it shows 'Submit' and 'Approve'
2. Hierarchy Approval > Shows 'Submit', 'Approve' in R12 > where as in 11i it shows 'Submit', 'Forward' and 'Approve'
3. Forward Approval using Approval Window > Shows 'Submit', 'Approve' in R12 > where as in 11i it shows 'Submit', 'Forward' and 'Approve'
4. Forward Approval using Documents Form > Shows 'Submit', 'No Action' and 'Approve' in R12 > where as in 11i it shows 'Submit', 'Forward', 'No Action', 'Forward' and 'Approve'
