Duplicate Notes Are Created In Service Request Form When Pressing Save Multiple Times

On : 12.1.3 version, Service Request Form
When adding notes in the SR form by pressing Control and S keys multiple times duplicate notes are getting created.
One press of Control and S keys should save a note and further presses should not add any new changes
Steps to Replicate
The issue can be reproduced at will with the following steps:
1. SR duplication check turned on when creating a SR.
2. Write a note
3. Control and S keys are pressed and the note is saved
4. Further Control and S key presses cause the duplication of the note again and again
