Supplier Administrator Cannot Create User Account for Supplier Contact Due to Error ORA-01427 Indicating Subquery Returns More Than One Row

In 12.1.3 iSupplier Portal
Supplier administrator cannot create user account for supplier contact due to error ORA-01427 indicating subquery returns more than one row. This error occurs for a particular Supplier contact. Based on the symptoms it seems there may be a duplicate of this contact in the HZ tables (hz_parties, hz_relationships) but no way to remove the duplicate from the user interface.

STEPS TO REPRODUCE
1. Navigate to Purchasing > Supply Base > Suppliers
2. Query for supplier CDW Direct
3. Go to Contact Directory - One contact available.
4. Check the checkbox to create user account for this contact, and click Apply
- get the error
This supplier contact needs to have a User Account created, but the user account cannot be created because of this error that occurs.
