Symptoms
When try to modify or enter new values in Accounting Flexfield Value sets after R12 upgrade, the following error occurs:
ERROR

ORA-01403: no data found
FRM-40735: ON-INSERT trigger raised unhandled exception ORA-04045.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Login to System Administrator
2. Navigate to Application > Validation > Values
3. Query the value set and try to enter a new value
