oc4j_socket_recvfull Error While Creating Change Order After DB and IE11 Upgrade (Doc ID 1987502.1)

Symptoms

Internal Server error found in the  creating Change order  

Getting Internal server error and below error in Apache when upgraded DB Version 11.2.0.4 and Appplying Patches for IE11

Note: Sometimes the error does not occur on the click on Save and Continue but on the click on 'Apply' button which comes after saving the CO.

Error
File does not exist: /u01/apps/R12/XXXXX/XX/apps/XXXXXXXX_XXXX/portal/YYY
mod_oc4j: Failed to find a failover oc4j process for session request for destination: application://oacore (no island or jgroup).


Steps
1. Login to Ebusiness application
2. Go under 'Change Management' and click on 'Create Change Order'
3. Enter name and description of change order and add an item by clicking on Add 5 rows.
4. Click on 'Save and Continue'.
5. Get the internal error
