PO Approval Workflow Performance Issue After Upgrading the Application and Database

Symptoms
After an upgrade users are reporting slow performance when submitting Purchase Orders (PO)s for approval. The performance has gone from approving in a few seconds to taking almost 5 minutes per document.
The Analyzer output from <Note 1525670.1> - R12: PO Approval Analyzer Diagnostic Script shows the following:

Although the elapsed times in the above output were not significant, it was noted that the documents were moving to BACKGROUND rather than approving immediately.
Changes
Upgrade the application from 12.0.6 to 12.1.3 and the database from 10.2.04 to 11.2.0.4.
