1099R - Federal / Combined File Test File Rejected by IRS Because 'K' Record Totals do not Equal 'B' Record Totals

Find that the 1099R - Federal / Combined File is rejected by the IRS because the reported that the 'K' records contain control totals that do not equal the IRS total of the 'B' records that have the same Fed/state code in positions 747/748.
Expect the file to be correct.
The issue can be reproduced at will with the following steps:
Run the Federal 1099R Magnetic Report
A test 1099-R file was sent to the IRS. The reported that the 'K' records contain control totals that do not equal the IRS total of the 'B' records that have the same Fed/state code in positions 747/748.
After looking further at the file, it appears that certain people are receiving two 'B' type records for a given payer/state.
The first "B" record is reporting the YTD totals correctly, but the State Code (pos 747-748) is blank.
The second 'B' record (for the same payer) has all '0's in the amount fields and has the State Code field populated with the correct state code.
The first record with the amounts populated should have the State Code populated on that record. We are not sure why a second record with 0 amounts is being populated.
