After Applying 19258579, Layout Of Login Window Which Appears After You See "Your log on session is no longer valid." Is Incorrect (Doc ID 1996120.1)

Symptoms

After Forms Session timeout occurs, you see the screen "Your log on session is no longer valid. Would you like to log back in so you can continue working? If you do not log in, any outstanding data change in your forms will not be saved."

If you answer "YES", a new browser window for Login shows up. The layout of this Login window is incorrect.

Note: The layout of initial Login window is correct only the Login window after timeout is incorrect. The defect occurs on IE/Firefox/Chrome.

Changes

This defect occurs after you login to the EBS with "Accessibility" as "Screen Reader Optimized". Such a login changes "Self Service Accessibility Features" profile to "Screen Reader Optimized" in the user's level.

Additionally, the defects occurs only on an environment which applied Patch:19258579(ORACLE APPLICATIONS RELEASE 12.1: CPU PATCH FOR OCT 2014).

Patch:19873049(ORACLE APPLICATIONS RELEASE 12.1: CPU PATCH FOR JAN 2015) or <Document 1348791.1> (R12: Font And Links Have Changed After Patching) did not solve the problem.
