Can't assign responsibility as the forms responsibility does not show on Security>User Define form.

Problem Description
When trying to assign custom responsibility in forms, the responsibility does not show on the Security>User Define Form and does not show in the Responsibility>Define form when querying on it.

If queried in the database, it shows.

This can be re-produced with the following steps:
1. Assign a custom responsibility in forms.
2. Attempt to review the assigned responsibility.
