Symptoms
When attempting to set up checklists in Oracle HRMS, a Life Event needs to be created.
When opening the Life Event Reasons form
the following error occurs.

Invalid argument to HR_form.define: p_blockname='LEE'
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Global HRMS->Total Compensation->General Definitions->Additional Setup->Life Event Reasons.
2. Error when form tries to open
