E-Business Suite R12 Integrated SOA Gateway, Installation related issues

User attempts to access any web service in the Integration repository fails with error "404 Not found" in every alternate invocation.

Similarly, test query URL (http://<HostName>.<DomainName>:<Port#>/webservices/SOAProvider/?testquery) returns "404 Not found" in every alternate invocation.
