PO Action History is Missing the APPROVE record (Doc ID 2012815.1)

Symptoms

On : 12.2.3 version

After submitting a change on a Purchase Order (PO), the PO Action History is not showing an Approve action when using the standards approval workflow.  For example the action history displays:

Sequence   Action      Revision
0               Submit   0
1               Submit   1

The expected functionality would be to display a Submit & Approve action in the Action History.
