Symptoms

After upgrade from 11.2.0.3 to 12.1.0.2 database along R12.2, autoconfig errors with:

AutoConfig could not successfully execute the following scripts:
  Directory: /u01/Vision/applvis/fs1/inst/apps/VISION/admin/install
  jtfictx.sh INSTE8_PRF


The issue can be reproduced at will with the following steps:
1. Install R12.2 and then upgrade it's database with 11.2.0.3 to 12.1.02.
2. Completed all upgrade cycle successfully and autoconfig.sh ran successfully on the database, but failed on the Application tier.

The issue has the following business impact:
Due to this issue, users cannot complete the upgrade steps.
