PO Approvals Workflow Ends In Error Using AME

Symptoms
When attempting to approve the PO using AME,
the following error occurs in the approval workflow

AME_PROCESS_APPROVER_APPRV (Update Approval List for Approve using AME) #EXCEPTION 100 ORA-01403: no data found ORA-01403: no data found Wf_Engine_Util.Function_Call(PO_AME_WF_PVT.PROCESS_RESPONSE_APPROVE, POAPPRV, 83003-124005, 252576, RUN)
The issue can be reproduced at will with the following steps:
1. Configure PO approvals using AME.
2. Create Purchase order and submit for approval.
3. First approver approves the PO and notified to second approver.
4. Second approver approves the PO and PO approval workflow ends with above error.
