INCOIN Sometimes Fails With INV_IOI_ERR On ITEM_CATALOG_GROUP_ID

"We are trying to convert items to a different catalog. All master items successfully converted, but only a small number of organization items has converted successfully. Item Import is failing with a warning for column Item_Catalog_Group_Id with no error message.. The message name is INV_IOI_ERR for MTL_SYSTEM_ITEMS_INTERFACE table. The remaining interface records have set_process_id = 3. There are no missing item numbers ,descriptions, or duplicates and the data generally matches other successful imports for these catalog conversions. We were able to process items of the same nature the prior Friday evening and the few days after the occurrence of the warnings. Only 3 batches are giving us the same warning out of 9.
Our master org is xxx, and our child org is yyy. Both xxx and yyy items were in the same catalog group prior to the import. I confirmed this when running other test. The xxx items imported successfully, but only a handful of the yyy items did as we were updating the catalog group. Therefore, majority of the LAF items are not in the same catalog as the xxx after the error. The import was to "convert" items into PIM."
"We created items in catalog zzz and the items are not in PIM. Now we need to "add" the items to PIM, but to do so we need to assign a new catalog of Rings. So we populated the interface with the items as the new catalog Rings and ran the import. The import is then failing for unknown error.
We do this daily for similar sets of items and they do not fail the import. Clearing the interface and trying again still results in the same error.
Right now the import is not failing for other items while "converting" them to Rings and they originated from the same catalog zzz. There is something within these items causing the error, but there is nothing from the import logs that help. The data essentially matches what goes through today with the exception of unique values."

The issue can be reproduced at will with the following steps:
1. Populate MTL_SYSTEM_ITEMS_INTERFACE.
2. Run INCOIN.
3. Observe the results, both positive and negative.
