Unable To Switch User In Worklist

Symptoms
On : 12.2.4 version, Workflow Administration
Scenario 1 : When attempting to switch worklist the following errors can occur
ERROR
Error: Access to <Username> Worklist has been revoked.
Error: You are trying to access a page that is no longer active.
- The referring page may have come from a previous session. Please select Home to proceed.
Scenario 2 :
Switching user with users having an @ symbol in their Username was causing an error for which customer applied patch 20924472 which allows the usage of @ in the username.
Post that switch user caused the below error while accessing the worklist
ERROR
oracle.apps.fnd.framework.OAException: java.sql.SQLException: ORA-01422:
exact fetch returns more than requested number of rows
ORA-06512: at "APPS.WF_NOTIFICATION", line 5227
ORA-06512: at line 1
