Symptoms

Using the Oracle Applications Release 12.1.3, on the 'Security:User > Define' form, adding A Responsibility Fails with ORA-01685 error:

ORA-20002 WF_INVAL_USER_ROLE encountered during execution
of Rule function WF_ROLE_HIERARCHY.Cascade_RF for event
oracle.apps.fnd.wf.ds.userRole.created with key:
PER:60491 FND_RESP:52709|2457219:52008
has been detected in FND_USER_RESP_GROUPS_API.INSERT_ASSIGNMENT
ORA-01685: Max extents (505) reached in index APPLSYS.WF_USER_ROLE_ASSIGNMENTS_N1 partition FND_RESP
