Symptoms

Error when adding party association to an instance via the Install Base UI the following error occurs:

ORA-01422: exact fetch returns more than requested number of rows in Package CSI_PARTY_RELATIONSHIPS_PVT Procedure CREATE_INST_PARTY_RELATIONSHIP

Steps to Reproduce
The issue can be reproduced at will with the following steps:
1. Assign a contact to the address book on supplier in Payables which has multiple Sites
2. Assign this vendor contact to install base using the Associations tab
3. Multiple rows displayed for only one Contact attached to a Vendor source in Associations tab
4. Attempting to update one of the contacts rows displayed results in ORA-01422 error
