ORA-01403 Error When Saving Exceptions on the Service Availability Form

When attempting to populate the Service Availability form with the Item value and corresponding Product Exceptions and saving. the following error is thrown:
Error ORA-01403: no data found
FRM-40735: KEY-COMMIT trigger raised unhandled exception ORA-01843.
1. Using the Service Contracts Manager responsibility
2. Open the Service Availability form.
3. Input an Item and Product Exceptions and save.
4. Receive the indicated errors.
