R12: AP/ZX: Despite Setting Up A Legal Authority As Per Note 817698.1, The Tax Authority LOV In The Withholding Tax Details Form Contains No Values

Symptoms
In the Oracle eBusiness Suite's (EBS) Accounts Payables Withholding Tax Details form, the Tax Authority List of Values (LOV) contains no values. This is despite setting up a Legal Authority as per Note 817698.1.
It is expected that the Tax Authority should be populated with values.
The issue was reproduced with the following steps:
1. Set up a Legal Authority as per Note 817698.1.
1. With a Payables-enabled responsibility, navigate to Setup > Tax > Withholding > Code
2. Enter the data. In Tax Authority LOV, no values are returned.
Due to this issue it is not possible to create new withholding tax codes, or update the existent ones.
