Problem Statement:

On : 12.1.3 version, Document of Record

While using Document of Record with approvals, error message is received.

ORA-20002: 3133: ACTIVITY INSTANCE 'CREATE_BPRV_BLOCK'

The issue can be reproduced at will with the following steps:

  1. Create DOR >> Error is received
