Error "The webpage cannot be found HTTP 400" When Creating a New Questionnaire Using HR Professional V4.0 Responsibility

On : 12.1.1 version, Setup
When creating a Questionnaire, the following error occurs.
The webpage cannot be found HTTP 400
Most likely causes:
There might be a typing error in the address
If you clicked on a link, it may be out of date.
The issue can be reproduced at will with the following steps:
1. Log into Applications.
2. Navigate as HR Professional V4.0 > Questionnaire Administration > Create new Questionnaire
3. Enter HTML text. Click Continue
4. Click on "Apply" button. Then the above error is shown.
