R11i: R12: OIE: "Pending System Administrator Action" Workflow Error Occurs if More than One FND User Per Employee

Expense Reports are stuck in workflow with "Pending System Administrator Action" when multiple FND users are attached to the same employee. For example, one Employee has three FND Oracle User id's pointing to same Employee. Navigate is System Administrator: Security=>User=>Define How can this be resolved?
