On 12.1 HRMS RUP7 version of the Benefits Confirmation and Summary Report,

The life event occurred date is incorrect in the body of the generated Benefits Confirmation and Summary Report output.

Expect the life event occurred date referenced to reflect "01-AUG-2015" (or the current life event date).

The issue can be reproduced at will with the following steps:
1. Go to responsibility US Super HRMS Manager.
2. Navigate to Processes and Reports > Submit Processes and Reports > Single Request.
3. Choose Request Name "Benefits Reports Wrapper Process" and Report Name "Benefits Confirmation and Summary Report".
4. Submit the request and view the output.
